Pathway Humanity’s Therapeutic Housing Program is actively seeking a compassionate and dedicated Case Manager to join our team in providing essential housing and mental health services to homeless and low-income individuals struggling with substance abuse and mental health disorders.

Key Responsibilities

  • Crisis Intervention: Respond promptly and effectively to emergency situations involving program participants, ensuring their safety and well-being

  • Trauma-Informed Care:  Cultivate and maintain a team culture that is sensitive to the experiences of trauma, fostering an environment of respect, empathy, and understanding for both clients and colleagues

  • Data Management:  Accurately and efficiently manage client data within the Homeless Management Information System (HMIS), ensuring compliance with program requirements and data privacy standards

  • Program Training:  Provide comprehensive training to staff and clients on program standards, policies, and procedures, promoting a clear understanding of expectations and responsibilities

  • Professional Development:  Stay informed about current best practices and emerging trends in the field of housing and mental health services through ongoing research, training, and professional development opportunities

  • Collaboration and Communication:  Actively participate in team meetings, interdisciplinary case conferences, and community collaborations to ensure coordinated and effective service delivery

  • Problem-Solving and Advocacy:  Identify and address barriers to client success, advocating for their needs and connecting them with appropriate resources and support services

  • Organizational Alignment:  Uphold and promote the mission, values, and goals of Pathway Humanity, contributing to a positive and supportive organizational culture

Qualifications

  • Relevant Education and Experience:  A Bachelor’s degree in Social Work, Psychology, or a related field is preferred; relevant experience in case management, social services, or mental health services is required

  • Knowledge and Skills:  Demonstrated knowledge of trauma-informed care principles, substance abuse and mental health disorders, and housing resources; strong communication, interpersonal, and problem-solving skills; proficiency in HMIS and other relevant software applications

  • Personal Qualities:  Compassion, empathy, and respect for individuals from diverse backgrounds; cultural sensitivity and awareness; commitment to social justice and equity; ability to work effectively in a team environment and independently
